Job summary
Matrix Medical Network is seeking a licensed Nurse Practitioner to perform comprehensive in-home health assessments. You will review medical history, medications, and social determinants to identify care gaps and improve outcomes.
Travel to members’ homes or facilities is required, with support from a national network of clinicians. We offer flexible scheduling, competitive pay based on completed assessments, mileage reimbursement, and opportunities for licensure support and career advancement.
Qualifications
- Master’s Degree or commensurate experience and NP licensure
- Current NP licensure in state of practice with prescriptive authority
- AANP/AACN/ANCC board certification in NP specialty
- BLS/ACLS/CPR certification required or eligible
- 1 year NP experience preferred; new grads encouraged to apply
- Valid state driver’s license and auto insurance coverage
- Strong computer skills and familiarity with EMR/health records software
Responsibilities
- Conduct adult/geriatric health assessments including history, diagnoses, and education
- Order or administer point-of-care and lab-based testing as needed
- Collaborate with physicians, case managers, social workers, and care staff
- Provide health assessments and follow-up at the member’s home or care facility
