Matrix Medical Network is seeking a skilled Nurse Practitioner to join our team for in-home assessments. You will conduct comprehensive appraisals at the member’s residence, review medical history and medications, and coordinate with physicians and care teams to close care gaps.

Responsibilities include clinical assessments, ordering tests, and ensuring thorough documentation. This role offers flexibility, competitive pay, and a sign-on bonus, with travel to members’ homes across the country.

Qualifications

• Master’s degree in nursing and NP licensure in the state of practice.
• Board certification in Adult, Adult Gerontology, Family, or related NP specialty.
• Active BLS/ACLS/CPR certification.

Responsabilités

• Perform adult/geriatric assessments including medical history and documentation.
• Administer and order point-of-care tests and lab work as needed.
• Collaborate with physicians, case managers, social workers and caregivers.
• Coordinate with PCP for patient education and follow-up.
