Job summary

Matrix Medical Network seeks nurse practitioners to perform Comprehensive Health Assessments in members' homes or SNFs. Each visit lasts up to one hour, reviewing health, meds, history, and social factors to close care gaps.

The role offers flexible scheduling, Monday–Sunday coverage, competitive pay per assessment, and a $1,500 sign-on bonus. Prior NP licensure and board certification are required; EHR proficiency and travel within state are expected.

Qualifications

• Master’s degree in NP with current licensure in state of practice.
• Board certification in an NP specialty (Adult, Gerontology, Family, Emergency, or Acute Care).
• BLS, ACLS or CPR certification required.
• 1 year NP experience preferred; new grads encouraged to apply.

Responsibilities

• Conduct adult/geriatric assessments including medical history and treatment plans.
• Order and administer point-of-care and lab tests as needed.
• Collaborate with PCPs, case managers, social workers and family members.
• Provide care at members' residences including homes and SNFs.
• Support patient education and follow-up per Matrix guidelines.
