Job summary

Matrix Medical Network seeks experienced Nurse Practitioners to perform Comprehensive Health Assessments in patients’ homes or Skilled Nursing Facilities. Each visit lasts up to an hour and focuses on medical history, adherence, and social factors to identify care gaps and risks.

You will collaborate with PCPs, case managers, and staff to improve outcomes and coordinate follow-up. We offer flexible scheduling, competitive pay per assessment, mileage reimbursement, 401(k), and licensure support

Qualifications

• Master’s Degree in Nursing and NP licensure in state of practice.
• Current NP licensure withprescriptive authority or ability to obtain.
• Board certification in relevant NP specialty (Adult/Family).

Responsibilities

• Perform adult/geriatric health assessments in home or facility settings.
• Administer point-of-care and lab-based tests as needed.
• Collaborate with physicians, case managers, social workers and caregivers.
• Coordinate with PCP for education and follow-up.
• Deliver services at members' residences or SNFs.
