Job summary

Matrix Medical Network seeks highly skilled Nurse Practitioners to perform Comprehensive Health Assessments in the homes of members, including adults and geriatric populations. You will review medical histories, medications, and social determinants to identify care gaps and coordinate with PCPs.

These full-time roles offer flexible scheduling, reimbursement for mileage, and opportunities to support patients where they live, including homes and Skilled Nursing Facilities, across participating

Qualifications

• Master’s Degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice with prescriptive authority.
• Board certification in Adult, Adult Gerontology, Family or related specialty.
• Current BLS, ACLS or CPR certification.
• 1 year of NP experience preferred; new grads encouraged to apply.
• Valid state driver’s license for the state of residence.
• Automobile insurance coverage for the state of residence (or ability to obtain).
• Strong computer skills and familiarity with patient health/medical record software.
• Excellent verbal and written communication skills.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis, treatment, health education, referrals, follow-up, and documentation according to Matrix guidelines.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, family members and other care teams.
• Coordinate with the PCP on patient education and follow-up.
• Provide services at the member’s residence which can be in homes or Skilled Nursing Facilities.
