Job summary

Matrix Medical Network seeks licensed Nurse Practitioners to perform comprehensive in-home health assessments across the nation, focusing on adult and geriatric populations. Visits occur at the member's residence, with documentation and coordination with PCPs to close care gaps when appropriate.

Qualifications

• Must hold current NP licensure in the state of practice with prescriptive authority or ability to obtain.
• Board certification in Adult/Family NP specialty (AANP/AACN/ANCC) required.
• BLS, ACLS or CPR certification must be current.
• Minimum 1 year NP experience; new grads encouraged to apply.
• Valid state driver's license and auto insurance (or ability to obtain).
• Strong computer skills and experience with health records software.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis, treatment, health education, referrals, follow-up and documentation.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, family members and caregivers.
• Coordinate with PCP on patient education and follow up as necessary.
• Provide services at the member's residence which can be in homes or Skilled Nursing Facilities.
