UnitedHealth Group's Optum HouseCalls is hiring Advanced Practice Clinicians (Nurse Practitioners or Physician Assistants) to conduct in-home health assessments for Medicare Advantage members. This field-based role emphasizes education, gap closure, and collaboration with PCPs to improve outcomes.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active and unrestricted licensure in the state you will be practicing in.
• Driver’s license and access to reliable transportation for home visits.
• Ability to navigate varied home environments and perform assessments.
• If working in a state allowing independent NP/PA practice, obtain licensure approvals as required.

Responsibilities

• Conduct comprehensive in-home assessments for Medicare Advantage and other plan members.
• Document diagnoses and develop care management plans.
• Communicate findings to members’ PCPs to address care gaps.
• Recognize urgent/emergent situations and intervene if needed.
• Educate members on disease processes, medications, and adherence.
• Address social determinants of health and provide referrals.
