Job summary

UnitedHealth Group's Optum Home & Community Care is expanding its HouseCalls program and seeks Advanced Practice Clinicians (NPs or PAs) to perform in-home health assessments for Medicare Advantage members in a flexible, field-based role.

The position emphasizes education, gap closure, and collaboration with primary care providers, with no on-call, weekends, or holidays, and offers loan repayment and sign-on bonuses along with a comprehensive benefits package.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric or AGPC.
• Current NCCPA certification and state licensure for PAs.
• Active, unrestricted licensure in the practicing state without disciplinary actions.
• Driver's license and reliable transportation for home visits.
• Ability to work in varied home environments and perform assessments.
• Eligibility to practice independently where allowed; if not, apply within required timelines.

Responsibilities

• Conduct in-home health assessments for Medicare Advantage members.
• Review medical history, med reconciliation, vitals and exam findings.
• Document diagnoses for care management and treatment planning.
• Communicate findings to PCPs to address care gaps.
• Identify urgent situations and intervene appropriately.
• Educate members on diseases, medications, and adherence.
• Address social determinants of health and provide referrals.
