Job summary

UnitedHealth Group, through Optum Care, is seeking a HouseCalls Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health assessments for Medicare Advantage and other members. The role emphasizes education, gap closure, and coordination with primary care providers to improve outcomes.

The position offers a flexible, 8am–8pm scheduling window across a 7-day week, with no on-call, weekends, or holidays.

Qualifications

• Active ANCC/AANP certification in Family/Adult/Geriatric Primary Care
• NCCPA certification for PAs and state licensure
• Active, unrestricted license in the practicing state without disciplinary actions
• Active driver's license and reliable transportation for home visits
• Ability to navigate varied home environments and perform assessments
• Eligibility to practice independently where allowed and process alignment within 1–3 months

Responsibilities

• Conduct comprehensive in-home assessments including medical history, meds reconciliation, vital signs and physical exam
• Document diagnoses for care management and treatment planning
• Identify care gaps and communicate findings to PCPs
• Recognize urgent situations and intervene appropriately
• Educate members on diseases, medications and adherence
• Address social determinants of health and provide referrals as needed
