Job summary

UnitedHealth Group's Optum HouseCalls is hiring an Advanced Practice Clinician (NP/PA) to perform in-home health assessments for Medicare Advantage and other members. The role focuses on education, gap closure, and collaboration with PCPs, without prescribing duties.

The position offers flexible scheduling, a sign-on bonus for external candidates, and a performance-driven environment with opportunities for advancement within an interdisciplinary care team.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• Current NCCPA certification and state licensure
• Active, unrestricted licensure in the practicing state without disciplinary actions
• Active and unrestricted driver's license with access to reliable transportation
• Ability to navigate varied home environments and perform assessments

Responsibilities

• Conduct comprehensive in-home assessments including medical history review, medications, vital signs and physical exam
• Document diagnoses for care management and treatment planning
• Communicate findings to members' PCPs to address gaps in care
• Recognize urgent/emergent situations and intervene appropriately
• Educate members on disease processes, medications, and adherence; address social determinants of health

Skills

Clinical experience in family/geriatic

EMR proficiency

Communication with geriatric/Medicare

Languages besides English

Education

ANCC/AANP Family/Adult/Geriatrics certification

NCCPA certification and state licensure
