Job summary

Optum Home & Community Care is seeking a HouseCalls Advanced Practice Clinician (Nurse Practitioner) to perform in-home health assessments for Medicare Advantage and other members. You will operate in an interdisciplinary team to improve outcomes and coordinate with PCPs.

The role emphasizes non-prescribing fieldwork, education, gap closure, and collaboration with primary care providers, with flexible scheduling and a sign-on bonus.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active and unrestricted state licensure where you will practice.
• Driver's license and reliable transportation for home visits.
• Proficiency with electronic medical records and related tech.
• Fluency in languages other than English is a plus.

Responsibilities

• Conduct comprehensive in-home assessments for Medicare Advantage and other plan members.
• Document diagnoses for care management and create treatment plans.
• Communicate findings to primary care providers to close care gaps.
• Identify urgent situations and intervene appropriately.
• Educate members on diseases, medications, and adherence.
• Address social determinants of health and arrange referrals.
