This role emphasizes education, gap closure, and collaboration with primary care providers, with no prescribing duties and a field-based schedule.

Qualifications

• ANCC/AANP Family, Adult, Geriatric, or Adult-Gerontology Primary Care certification.
• NCCPA certification for PAs and state licensure.
• Active, unrestricted licensure in the practicing state.
• Active driver's license and reliable transportation for home visits.
• Ability to navigate varied home environments and perform assessments (sit/stand/kneel).
• If applicable, obtain licensure eligibility within required timelines.

Responsibilities

• Conduct comprehensive in-home assessments including medical history, medications, vital signs, and physical examination.
• Document diagnoses for care management and treatment planning.
• Communicate findings to members' PCPs to address gaps in care.
• Recognize urgent/emergent situations and intervene appropriately.
• Educate members on disease processes, medications, and adherence.
• Address social determinants of health and provide referrals as needed.
