Job summary

UnitedHealth Group seeks an Optum HouseCalls Advanced Practice Clinician (NP/PA) to perform in-home health assessments for Medicare Advantage members. This field-based role emphasizes education, gap closure, and collaboration with PCPs to improve outcomes.

Responsibilities include comprehensive exams, documentation, and referrals, with a focus on safe, quality care in patients' homes. Flexible scheduling supports work-life balance and may include travel within designated markets.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active, unrestricted licensure in the state you will be practicing in.
• Active and unrestricted driver's license and access to reliable transportation.
• Ability to navigate varied home environments and perform assessments.

Responsibilities

• Conduct comprehensive in-home assessments, including medical history review, medication reconciliation, vital signs, and physical exam.
• Identify and document diagnoses for care management and treatment planning.
• Communicate findings to PCPs to address gaps in care.
• Recognize urgent/emergent situations and intervene appropriately.
• Educate patients on disease processes, medications, and compliance.
• Address social determinants of health and provide referrals as needed.
