Job summary

UnitedHealth Group’s Optum HouseCalls team is seeking an Advanced Practice Clinician (Nurse Practitioner) to perform in-home health assessments for Medicare Advantage members and other plans. The role emphasizes education, gap closure, and collaboration with PCPs to improve health outcomes.

The position offers a flexible schedule (8am–8pm, 7 days), no on-call, weekends, or holidays, with a 10k sign-on for external candidates and potential language-driven incentives.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult‑Gerontology Primary Care.
• Active and unrestricted licensure in the practicing state (or ability to obtain by start date).
• Active and unrestricted driver's license with reliable transportation for home visits.

Responsibilities

• Conduct comprehensive in-home health assessments, including medical history, vitals, and physical exam.
• Document diagnoses for care management and treatment planning.
• Communicate findings to PCPs to address gaps in care and improve outcomes.
• Identify urgent/emergent situations and intervene appropriately.
• Educate members on disease processes, medications, and adherence.
• Address social determinants of health and provide referrals as needed.
