Job summary

UnitedHealth Group's Optum HouseCalls team seeks an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to conduct in-home health assessments for Medicare Advantage and other plan members. This field-based role offers flexible scheduling (8am-8pm, 20-40 hours weekly), no on-call, weekends, or holidays, and opportunities to collaborate with PCPs to improve health outcomes.

You will educate patients and address social determinants of health to support care planning.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• Active, unrestricted Missouri license (or ability to obtain by start date)
• Active and unrestricted driver's license and reliable transportation for home visits
• Ability to navigate varied home environments; sit/stand/kneel as needed to perform assessments

Responsibilities

• Conduct comprehensive in-home assessments including medical history review, vitals, and exams
• Document diagnoses for care management and treatment planning
• Communicate findings to PCPs to close care gaps
• Identify urgent situations and intervene appropriately
• Educate members on diseases, medications, and adherence
• Address social determinants of health and provide referrals as needed
