Job summary

UnitedHealth Group's Optum HouseCalls program seeks Advanced Practice Clinicians to conduct in-home health assessments for Medicare Advantage and other members. The role emphasizes education, gap closure, and collaboration with PCPs to improve outcomes.

Flexible scheduling is offered with no on-call, weekends, or holidays. Total compensation includes base pay plus bonus, with a comprehensive benefits package and equity options.

Qualifications

• Active ANCC or AANP certification for NP or NCCPA certification for PA and state licensure.
• Active and unrestricted driver''s license; reliable transportation.
• Ability to obtain licensure by start date if required.
• Ability to work in varied home environments; flexible mobility.
• Experience with Medicare Advantage populations is a plus.

Responsibilities

• Conduct comprehensive in-home assessments (medical history, vitals, exam).
• Perform screenings and point-of-care testing as appropriate.
• Document diagnoses for care management and treatment planning.
• Communicate findings to members'' PCPs to address care gaps.
• Identify urgent situations and intervene when needed.
• Educate members on diseases, medications, and adherence.
• Address social determinants of health and provide referrals.
