Matrix Medical Network seeks Nurse Practitioners to perform Comprehensive Health Assessments in members' homes across New York. Each visit lasts up to one hour and focuses on health history, medications, social determinants, and risk factors to close care gaps.

You’ll work with PCPs and care teams, with flexible scheduling, competitive pay per assessment, mileage reimbursement, and opportunities for state licensure support. Prior NP licensure and certifications are required.

Qualifications

• Must have NP licensure in state of practice with prescriptive authority
• Board certification in Adult, Gerontology, Family, or Acute Care nursing preferred
• BLS/ACLS or CPR certification required; 1 year NP experience preferred; new grads welcome

Responsabilités

• Perform adult/geriatric health assessments in patients' homes
• Review medical history, medications, and social environment
• Coordinate with physicians, case managers, and care teams
• Provide patient education and follow-up coordination
• Ensure documentation per Matrix guidelines and protocols
