Job summary

Matrix Medical Network seeks nurse practitioners to perform comprehensive health assessments in patients' homes or SNFs. Each visit lasts up to one hour and includes review of medical history, adherence, and risks, with reporting per Matrix guidelines.

Applicants should have NP licensure in their state, board certification, BLS/ACLS, and be comfortable traveling. We offer flexible scheduling, competitive pay per assessment, mileage reimbursement, and 401(k) options.

Qualifications

• Master’s Degree or commensurate experience and NP licensure
• Current NP licensure in state of practice with prescriptive authority
• AANP/AACN/ANCC board certification in applicable NP specialty
• Current BLS, ACLS or CPR certification
• 1 year NP experience preferred; new grads welcome

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, referrals, and documentation per Matrix guidelines
• Administer and order point-of-care and lab-based testing as needed
• Collaborate with physicians, case managers, social workers, and family/caregivers
• Coordinate with PCP on patient education and follow-up
• Provide services at the member’s residence (home or SNF)
