Job summary

Matrix Medical Network seeks Nurse Practitioner providers to perform Comprehensive Health Assessments in members' homes or residential facilities. Each visit lasts up to one hour and informs care gaps and health outcomes.

Providers review medical history, medications, and social determinants, deliver education, and coordinate referrals with PCPs and case managers. A valid NP license and prescriptive authority are required.

Qualifications

• Master’s Degree or commensurate experience and satisfactory completion of NP licensure.
• Current NP licensure in state of practice with prescriptive authority or ability to obtain.
• AANP, AACN, or ANCC board certification in a relevant NP specialty.

Responsibilities

• Conduct adult/geriatric health assessments and document findings.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, and other health professionals to coordinate care.
