Job summary

Matrix Medical Network seeks a licensed Nurse Practitioner to perform comprehensive in-home health assessments. You will review medical history, medication adherence, and care needs, and collaborate with physicians and care teams to close gaps in care.

Responsibilities include ordering tests, documenting findings, and providing patient education. Travel to members' residences, including homes and facilities, is part of the role; licensing and prescriptive authority are required.

Qualifications

• Master’s degree in Nursing/NP program and NP licensure in state of practice.
• Board-certified in Adult/Family/Gerontology or related acute care specialties.
• Current BLS/ACLS or CPR certification.
• Minimum 1 year NP experience preferred; new grads encouraged.

Responsibilities

• Perform adult/geriatric health assessments including medical history and diagnoses.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers and other care team members.
• Provide patient education and follow-up with PCP when appropriate.
• Offer services at patients' residences, including homes or SNFs.
