Job summary

Matrix Medical Network seeks a Nurse Practitioner to perform comprehensive in-home health assessments for members. Responsibilities include reviewing medical history, medications, and social factors, with visits at patients’ homes or Skilled Nursing Facilities.

The role requires NP licensure with prescriptive authority, board certification, and strong communication; flexible scheduling and potential full-time opportunities are offered.

Qualifications

• Master’s degree required for NP licensure.
• Current NP licensure in state of practice with prescriptive authority or eligibility.
• Board certification in Adult, Adult Gerontology, Family, Emergency, or Acute Care.
• Active BLS/ACLS/CPR certification.
• One year NP experience preferred; new grads welcome.
• Valid driver’s license for state of residence.
• Auto insurance coverage required for state of residence.
• Strong computer skills and familiarity with EMR/health records software.
• Excellent verbal and written communication skills.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis, treatment, education and referrals.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers and other care team members.
• Coordinate with PCP on patient education and follow-up.
• Provide services at the member’s residence (home or SNF).
