Job summary

Matrix Medical Network is seeking a Nurse Practitioner to perform comprehensive in-home health assessments, reviewing medical history, medication adherence, and social factors to close care gaps. Visits occur at members’ residences, including homes or skilled nursing facilities.

The role emphasizes collaboration with physicians, case managers, and caregivers, with licensure and prescriptive authority required.

Qualifications

• Master’s Degree or commensurate experience and satisfactory completion of NP licensure.
• Current NP licensure in state of practice with prescriptive authority or ability to obtain as needed.
• AANP, AACN, or ANCC board certification in Adult, Adult Gerontology, Family, Emergency, or Acute Care.
• Current BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads encouraged to apply.
• Valid state driver’s license for the state of residence.
• Automobile insurance coverage for the state of residence (or ability to obtain) with required limits.
• Strong computer skills and familiarity with health records software.
• Excellent verbal and written communication skills.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnoses, and treatments.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers, and caregivers.
• Coordinate with PCP for patient education and follow-up.
• Provide services at members’ residences, including homes or Skilled Nursing Facilities.
