Job summary

Matrix Medical Network is seeking a qualified nurse practitioner to conduct comprehensive in-home health assessments. You will review medical history, medications, and social factors to identify care gaps and create individualized plans.

Travel to members' homes and SNFs is required, with flexible scheduling based on availability. The role offers competitive compensation, mileage reimbursement, and benefits.

Qualifications

• Must hold an NP license and prescriptive authority in the state of practice.
• Current NP board certification in Adult, Gerontology, Family, or related specialty.
• BLS/ACLS or CPR certification required.
• Typically 1 year NP experience; new grads may apply.
• Driver's license and auto insurance required for state of residence.

Responsibilities

• Perform adult/geriatric health assessments including medical history and treatment plans.
• Administer point-of-care and lab tests when needed.
• Collaborate with physicians, case managers and caregivers to coordinate care.
• Provide services at the member’s home or at Skilled Nursing Facilities.
