Job summary

Matrix Medical Network seeks a Nurse Practitioner to conduct comprehensive in-home health assessments. You will review medical histories, medications, and risks, documenting findings per Matrix guidelines, with visits at members' homes or SNFs.

You’ll collaborate with physicians, case managers, and other care team members to close care gaps and improve outcomes. A valid NP license and prescriptive authority are required, with flexible scheduling and field-based work across the region.

Qualifications

• Master’s Degree in Nursing and NP licensure in state of practice.
• Current NP licensure with prescriptive authority or ability to obtain.
• Certifications: AANP, AACN, or ANCC in Adult/Family/Gerontology specialties.
• BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads encouraged to apply.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis, treatment, education, referrals, and documentation per Matrix guidelines.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with PCP, case managers, social workers, and other care team members; coordinate patient education and follow-up.
• Provide services at the member’s residence (home or Skilled Nursing Facility).
• Document findings clearly and consistently in medical records and Matrix protocols.
