Job summary

Matrix Medical Network seeks a NP professional to perform comprehensive in-home health assessments. Each visit may last up to an hour and includes review of medical history, medications, and risk factors, with detailed documentation and follow-up as directed by Matrix guidelines.

The role emphasizes collaboration with physicians, case managers, and social supports, with care delivered at members’ homes or Skilled Nursing Facilities.

Qualifications

• Master’s Degree or commensurate experience and NP licensure
• Current NP licensure in state of practice with prescriptive authority or ability to obtain
• Board certification in Adult/Adult Gerontology Family or Emergency specialties

Responsibilities

• Conduct adult/geriatric assessments including medical history and diagnosis
• Administer and order point-of-care and lab testing as needed
• Collaborate with physicians, case managers, and other health professionals
• Provide home-based health assessments at the member’s residence or SNF
• Support patient education and follow-up with PCPs
