Job summary

Matrix Medical Network is hiring NP providers to conduct comprehensive in-home health assessments lasting up to an hour. You will review health history, medications, and risks to deliver insights that improve care gaps and outcomes, often coordinating with PCPs and care teams.

The role emphasizes autonomy and patient-centered care in home settings. We offer flexible scheduling and a strong compensation model with sign-on bonuses, plus benefits such as mileage reimbursement and 401(k).

Qualifications

• Master’s Degree or commensurate experience with NP licensure.
• Current NP licensure in state of practice with prescriptive authority if needed.
• AANP, AACN, or ANCC board certification in suitable NP specialty.
• Current BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads encouraged to apply.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, referrals, and documentation.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, family members, and caregivers as appropriate.
• Provide services at the member’s residence, including homes or Skilled Nursing Facilities.
• Coordinate with PCP for patient education and follow-up.
