Job summary

Matrix Medical Network is seeking Nurse Practitioners to perform comprehensive in-home health assessments for members in their homes or Skilled Nursing Facilities. Each visit focuses on medical history, current health status, medications, and overall well-being to identify care gaps and improve outcomes.

Providers conduct adult/geriatric assessments, order tests as needed, and coordinate with PCPs and care teams.

Qualifications

• Master’s degree and NP licensure in state of practice.
• Currently board-certified in Adult, AGPC or Family/Acute Care specialties.
• BLS/ACLS/CPR certification is required.
• 1 year NP experience preferred; new grads encouraged to apply.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, referrals and documentation per Matrix guidelines.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, family members, and other healthcare professionals.
• Coordinate with the Primary Care Physician on patient education and follow-up.
• Provide services at the member's residence (homes or Skilled Nursing Facilities).
