Job summary
Matrix Medical Network seeks Nurse Practitioners to perform Comprehensive Health Assessments in members' homes or Skilled Nursing Facilities. Each visit lasts up to one hour, reviewing health history, medications, and risks to close care gaps.
Responsibilities include adult/geriatric assessments, testing, and coordination with PCPs and care teams, with documentation per Matrix guidelines. We offer flexible scheduling and a generous sign-on bonus.
Qualifications
- Master’s Degree plus NP licensure in state of practice.
- Current NP licensure and prescriptive authority or ability to obtain.
- Board certification in Adult/GER/Family NP specialties.
- BLS/ACLS/CPR certification current.
- 1 year NP experience preferred; new grads welcome.
- Valid state driver’s license with automobile insurance.
- Strong computer skills and familiarity with medical records software.
Responsibilities
- Perform adult/geriatric health assessments with medical history and documentation.
- Administer point-of-care and lab testing as needed.
- Collaborate with physicians, case managers, social workers, and caregivers.
- Coordinate with PCP on patient education and follow-up.
- Provide services at member homes or Skilled Nursing Facilities.
