Job summary

Matrix Medical Network seeks a Nurse Practitioner to perform Comprehensive Health Assessments in patients’ homes or SNFs nationwide. You will review medical histories, medications, and social factors, coordinate with physicians and care teams, and provide education and follow-up care.

Responsibilities include ordering tests, documenting outcomes, and traveling up to 80-90% with overnight stays when required. Flexible scheduling complements a meaningful, impact-driven role.

Qualifications

• Master’s Degree or commensurate experience and satisfactory completion of NP licensure
• Current NP licensure in state of practice to include prescription authority or ability to obtain prescriptive authority
• AANP, AACN, or ANCC board certification in relevant NP specialty
• Current BLS, ACLS or CPR certification
• 1 year of NP experience
• Ability to travel up to 80-90% of the time including overnight travel
• Valid state driver’s license and automobile insurance in residence state

Responsibilities

• Conduct adult/geriatric health assessments including medical history, diagnoses, and treatment
• Perform point-of-care and lab-based testing as needed
• Collaborate with physicians, case managers, social workers, and caregivers
• Educate patients and follow up with care plans with PCPs
• Provide in-home or SNF-based health assessments and documentation
