Matrix Medical Network is seeking Nurse Practitioners to perform Comprehensive Health Assessments (CHA) in members’ homes or SNFs. You will review medical history, diagnoses, medications, and lifestyle factors to identify gaps in care and coordinate follow-up with physicians and care teams.

This role offers flexible scheduling, with visits from 8 am–7 pm on Sundays through Saturdays, and potential to increase earnings by expanding availability and visiting members beyond your home base.

Competenze

• Master’s degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice (prescription authority).
• Board certification in Adult, Gerontology, Family or related NP specialties.

Mansioni

• Conduct adult/geriatric assessments including medical history, diagnosis, treatment and education.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with PCPs, case managers and other health professionals; document findings per Matrix guidelines.
• Provide services at the member’s residence (home or Skilled Nursing Facility).
