Job summary

Matrix Medical Network is hiring nurse practitioners to perform in-home Comprehensive Health Assessments (CHA) across the nation. As an NP, you will review medical history, medications, and social factors during visits at the member's home or SNF, supporting care gaps closure.

Expect flexible scheduling (Sun–Sat, 8am–7pm) with competitive pay based on assessments completed ($90–$115 per assessment). Full-time opportunities and licensure support are available to help you grow your career.

Qualifications

• Master’s Degree or commensurate experience with NP licensure.
• Current NP licensure in state of practice with prescribing authority.
• Board certification in an NP specialty (Adult/Family/Gero/ACNP/etc).
• BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads encouraged to apply.
• Valid state driver’s license and auto insurance coverage as required by home state.
• Strong computer skills and familiarity with health IT/EMR systems.
• Excellent verbal and written communication skills.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, referrals, and documentation per Matrix guidelines.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, families, and other care team members.
• Coordinate with PCP on patient education and follow-up as necessary.
• Provide services at the member’s residence which can be in homes or Skilled Nursing Facilities.
