Matrix Medical Network is seeking a Nurse Practitioner to perform in-home Comprehensive Health Assessments (CHA) for adults, including geriatrics, at members' homes or SNFs. You will review medical history, medications, and social factors to identify care gaps.

The role emphasizes collaboration with PCPs, lab testing when needed, and documentation per Matrix protocols. A valid NP license and prescriptive authority are required, with opportunities for full-time advancement.

Competenze

• Master’s Degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice with prescriptive authority as needed.
• Board certification in Adult, Adult Gerontology or Family NP specialties.

Mansioni

• Conduct adult/geriatric assessments including history, diagnosis and treatment, education, referrals, follow-up and documentation per Matrix guidelines.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers, family members and other health professionals.
• Coordinate with PCP on patient education and follow-up.
• Provide services at members' residences, including homes or Skilled Nursing Facilities.
