Job summary

Matrix Medical Network seeks an experienced Nurse Practitioner to perform Comprehensive Health Assessments (CHA) in members' homes, sometimes in Skilled Nursing Facilities. You will review medical histories, assess health, education needs, and coordinate with care teams.

Expect up to 80-90% travel, with flexible scheduling and potential FT applicability. We offer competitive compensation, benefits, and support for licensure expansion across states.

Qualifications

• Must hold NP licensure in state of practice with prescriptive authority, or ability to obtain
• Board certification in one of Adult/Adult Gerontology/Family categories (AANP/AACN/ANCC)
• Current BLS, ACLS or CPR certification
• Minimum 1 year NP experience
• Willingness to travel up to 80-90% including overnight stays
• Valid state driver’s license and automobile insurance
• Strong computer skills and familiarity with health/medical record software
• Excellent verbal and written communication skills

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis, treatment, education, referrals, and documentation
• Administer and order point-of-care and lab-based testing as needed
• Collaborate with physicians, case managers, social workers, and family members
• Coordinate with PCP on patient education and follow-up
• Provide services at member homes or Skilled Nursing Facilities
• Travel to assigned states and neighboring regions as required
