Job summary

Matrix Medical Network seeks NP providers to perform Comprehensive Health Assessments (CHA) in the homes of older adults and at-risk individuals. Each visit lasts up to one hour and informs care gaps with insights into health history, med adherence, and environment.

Providers order tests, collaborate with PCPs, and document findings according to Matrix guidelines. Flexible scheduling includes weekends; full-time roles offer sign-on bonuses and licensure support.

Qualifications

• NP licensure in state of practice with prescriptive authority or ability to obtain
• AANP/AACN/ANCC board certification in Adult, Gerontology, Family, Emergency or Acute Care
• BLS/ACLS/CPR certification
• 1 year NP experience preferred; new grads encouraged to apply
• Valid state driver’s license and automobile insurance (or ability to obtain)

Responsibilities

• Conduct adult/geriatric assessments including history, diagnosis, treatment, and education
• Administer/order point-of-care and lab-based testing as needed
• Collaborate with physicians, case managers, social workers, and caregivers; document per Matrix guidelines
• Coordinate with PCPs for patient education and follow-up
• Provide services at the member’s home or Skilled Nursing Facility
