Job summary

Matrix Medical Network is seeking Nurse Practitioners to perform in-home Comprehensive Health Assessments (CHA) for members across the United States. Each visit focuses on health history, medication adherence, and social factors to reveal gaps in care and improve outcomes.

Providers work in members’ homes or SNFs with scheduling based on availability. The role offers competitive compensation, travel opportunities, benefits, and ongoing professional development.

Qualifications

• Master’s degree or commensurate experience with NP licensure.
• Current NP licensure in state of practice with prescriptive authority.
• AANP, AACN, or ANCC board certification in an adult/gerontology/family specialty.
• Current BLS, ACLS, or CPR certification.
• Valid state driver’s license and auto insurance coverage.

Responsibilities

• Conduct adult/geriatric health assessments including medical history, diagnoses, and treatment per Matrix guidelines.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, and family members to coordinate care.
• Provide in-home or residence-based health assessments and follow-up education with PCP coordination.
