Matrix Medical Network seeks a Nurse Practitioner to perform Comprehensive Health Assessments (CHA) in members' homes, reviewing health history, medications, and risks to improve care gaps. Providers assess health status and coordinate with physicians, case managers, and family members to ensure quality outcomes.

Visits can last up to one hour, with scheduling Sunday–Saturday from 8 am–7 pm, offering flexible day, evening, and weekend options based on availability.

Competenze

• Master’s Degree in Nursing (MSN) required or commensurate experience.
• Current NP licensure in state of practice with prescriptive authority if needed.
• Board certification in Adult, Adult Gerontology, Family, Emergency, or Acute Care.
• Current BLS/ACLS certification.

Mansioni

• Conduct adult/geriatric assessments including medical history, diagnosis, treatment, and education.
• Administer/order point-of-care and lab-based testing as needed.
• Collaborate with PCPs, case managers, social workers, and caregivers.
• Provide in-home services at member's residence or SNFs.
• Document findings clearly following Matrix guidelines and protocols.
