Job summary

Matrix Medical Network seeks nurse practitioners to perform comprehensive in-home health assessments for members across the country. Visits typically last up to an hour and focus on health history, current medications, and social determinants. The CHA program aims to close care gaps and improve outcomes.

Providers work with PCPs, case managers, and other professionals, traveling to members’ homes or SNFs. Flexible scheduling supports weekend and evening shifts, with generous earning potential

Qualifications

• Master’s Degree or commensurate experience with NP licensure.
• Current NP licensure in state of practice; prescriptive authority as needed.

Responsibilities

• Conduct adult/geriatric health assessments including history, diagnosis, and education.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers and other staff to coordinate care.
• Provide services at members’ homes or in Skilled Nursing Facilities.
• Document findings clearly per Matrix guidelines.

Board certification (AANP/AACN/ANCC)

CPR/BLS/ACLS

1 year NP experience preferred

Valid driver’s license

Strong communication skills
