Job summary

Matrix Medical Network is seeking a licensed Nurse Practitioner to perform Comprehensive Health Assessments (CHA) in members' homes or Skilled Nursing Facilities. You will assess medical history, medications, and risk factors, and coordinate with physicians and care teams for optimal outcomes.

Qualifications

• Master’s degree or commensurate NP experience.
• Current NP licensure in the state of practice.
• Valid driver's license with vehicle insurance.

Responsibilities

• Perform comprehensive CHA assessments including medical history and risks.
• Review health history, medications, and social factors for each member.
• Order point-of-care and lab testing as needed.
• Collaborate with PCP and care team for follow up and education.
• Provide services at the member’s home or SNF as required.
