Job summary

Matrix Medical Network is seeking skilled Nurse Practitioners to perform comprehensive in-home health assessments across the state. Each visit lasts up to one hour, reviewing medical history, medications and social factors to identify care gaps and improve outcomes.

Applicants must hold an NP license in the state, board certification in approved specialties, and strong documentation and communication skills.

Qualifications

• Master’s Degree in Nursing with NP licensure and board certification.
• Current NP licensure in the state of practice with prescriptive authority.
• Familiarity with adult-gerontology primary care or related specialties.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnoses, treatment and follow-up.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with PCPs, case managers, social workers and families as appropriate.
• Provide services at the member’s residence (home or SNF) with proper documentation.
