Job summary

Matrix Medical Network is seeking skilled nurse practitioners to perform comprehensive in-home health assessments. You will review health histories, medications, and risks, delivering actionable insights to improve patient care at the member’s home or in SNFs.

The role offers flexible scheduling with visits across the week, comprehensive onboarding, and opportunities for professional growth. Prior NP experience and state licensure with prescriptive authority are preferred.

Qualifications

• Master’s degree and NP licensure in state of practice
• Current NP licensure with prescriptive authority
• Board certification in a relevant NP specialty
• Current BLS/ACLS or CPR
• 1 year NP experience preferred; new grads welcome

Responsibilities

• Conduct adult/geriatric health assessments in members' homes
• Review medical history, medications, social determinants and risks
• Coordinate with PCPs, case managers and family members; document clearly
• Order point-of-care and lab-based testing as needed
• Travel to members' residences including home or SNFs, providing education and follow-up
