Job summary

Matrix Medical Network seeks qualified NP providers to perform comprehensive in-home health assessments. Each visit lasts up to one hour and includes review of medical history, medications, and risks to identify care gaps and improve outcomes.

Providers administer tests as needed, collaborate with physicians and care teams, and deliver care at the member's home or skilled nursing facilities. Strong clinical knowledge and patient communication are essential.

Qualifications

• Master’s degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice.
• AANP, AACN, or ANCC board certification in relevant NP specialties.
• Current BLS, ACLS or CPR certification.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis, and treatment.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, and caregivers.
• Coordinate with PCP on patient education and follow-up.

Education
