Job summary

Matrix Medical Network is seeking Advanced Practice Providers to perform comprehensive in-home health assessments nationwide. This role offers flexible scheduling and a focus on proactive care for older adults and at-risk individuals.

You will perform comprehensive health assessments at patients' homes or SNFs, document findings, order tests as needed, and collaborate with PCPs, case managers, and family members to close care gaps.

Qualifications

• Must hold NP licensure and applicable state license.
• Current BLS/ACLS certification.
• Experience in adult/geriatric assessments preferred.

Responsibilities

• Conduct adult/geriatric assessments with documentation per Matrix guidelines.
• Administer point-of-care and lab tests as needed.
• Collaborate with physicians, case managers, social workers, and families.
• Provide services at member homes or Skilled Nursing Facilities.
• Coordinate education and follow-up with PCP.
