Job summary

Matrix Medical Network seeks a Nurse Practitioner to perform comprehensive in-home health assessments for older adults and at-risk individuals. You will consult with physicians and care teams to address medical needs and gaps in care in the member’s home environment.

Responsibilities include reviewing medical history, medication adherence, and patient education, while documenting findings per Matrix guidelines.

Qualifications

• NP licensed and board-certified in applicable adult care specialty.
• Current BLS/ACLS or CPR certification.
• Strong verbal and written communication skills.
• Valid driver’s license and auto insurance where required.

Responsibilities

• Conduct adult/geriatric assessments including history, diagnoses, and referrals.
• Administer point-of-care and lab testing as needed.
• Collaborate with PCPs, case managers, and family members.
• Provide care at members' homes or Skilled Nursing Facilities.
• Document encounters per Matrix guidelines.
