Job summary

Matrix Medical Network is seeking advanced practice providers to conduct comprehensive in-home health assessments across the nation. Assessments cover medical history, medications, and functional status, with documentation per Matrix guidelines.

Providers administer point-of-care testing, coordinate with physicians and care managers, and deliver patient education in the member's home or SNF settings. Flexible scheduling and travel are supported to improve access and outcomes.

Qualifications

• Master’s degree in nursing or equivalent.
• Current NP licensure in state of practice.
• Board certification in relevant NP specialty.

Responsibilities

• Perform in-home comprehensive health assessments including medical history and diagnoses.
• Administer point-of-care and lab-based testing as needed.
• Collaborate with PCP and care team; document findings per Matrix guidelines.
