Job summary

Matrix Medical Network seeks an experienced Nurse Practitioner to perform comprehensive in-home health assessments. You will review medical histories, coordinate care with physicians and case managers, and provide patient education in home or facility settings.

Must hold active NP licensure in the practice state, board certification, and be willing to travel extensively, including overnight stays. Competitive pay with sign-on bonus and flexible scheduling offered.

Qualifications

• Master’s Degree in Nursing or commensurate experience with NP licensure.
• Current NP licensure in state of practice with prescriptive authority as needed.
• AANP/AACN/ANCC board certification in applicable NP specialty.
• Current BLS/ACLS/CPR certification required.
• 1 year NP experience.
• Willingness to travel up to 80-90% and overnight stays as needed.
• Valid state driver’s license and auto insurance coverage.

Responsibilities

• Perform adult/geriatric assessments including history, diagnosis, treatment and education.
• Order/perform point-of-care and lab tests as needed.
• Collaborate with physicians, case managers and support staff.
• Provide patient education and follow-up in home or facility settings.
• Maintain documentation per Matrix guidelines and protocols.
