Job summary

Matrix Medical Network provides comprehensive in-home health assessments nationwide, improving care access for older adults and at‑risk individuals. Providers perform thorough health reviews during visits of up to one hour, review medical history, medications, and social factors, and order tests as needed.

Qualifications

• Master’s degree and NP licensure required.
• Active NP license in state of practice with prescriptive authority if needed.
• Board certification in listed NP specialties is required or expected.
• BLS/ACLS/CPR certification current.
• 1 year NP experience preferred; new grads encouraged to apply.
• Valid driver’s license for state of residence and automobile insurance as required.
• Strong computer skills and experience with health records software.
• Excellent verbal and written communication.

Responsibilities

• Conduct adult/geriatric assessments including history, diagnosis, and treatment.
• Administer and order point-of-care and lab tests as needed.
• Coordinate with physicians, case managers, social workers, and caregivers.
• Provide patient education and follow-up with PCPs as required.
• Deliver services at members’ homes or in Skilled Nursing Facilities.
