UnitedHealth Group’s Optum HouseCalls program seeks an Advanced Practice Clinician (NP/PA) to perform in-home health assessments for Medicare Advantage members. This field-based role emphasizes education, gap closure, and collaboration with primary care providers to improve outcomes.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active, unrestricted Texas license (or ability to obtain by start date).
• Valid driver's license and reliable transportation for home visits.
• Ability to navigate varied home environments and perform assessments.

Responsibilities

• Conduct comprehensive in-home assessments for Medicare Advantage and other members.
• Document diagnoses for care management and coordinate with PCPs.
• Educate patients on disease processes, medications and adherence.
• Identify urgent situations and intervene; address social determinants of health.
• Communicate findings to primary care teams to close care gaps.
