Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• Active, unrestricted Louisiana license by start date with no disciplinary actions
• Unrestricted driver's license with access to reliable transportation for home visits
• Ability to navigate varied home environments and perform assessments (sit/stand/kneel)

Responsibilities

• Conduct in-home assessments for Medicare Advantage and other plan members
• Document diagnoses for care management and treatment planning
• Communicate findings to members' PCPs to address care gaps
• Educate members on disease processes, medications, and adherence
• Address social determinants of health and provide referrals as needed
