Job summary

UnitedHealth Group’s Optum Home & Community Care is seeking an Advanced Practice Clinician (Nurse Practitioner) for HouseCalls. You will perform annual in-home health assessments for Medicare Advantage members, collaborating with PCPs to close gaps in care while focusing on education and outcomes.

This field-based role offers flexible scheduling (20–40 hours/week), no on-call or weekend requirements, and a sign-on bonus for external candidates.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Gerontic, or Adult-Gerontology Primary Care
• Active, unrestricted RN/NP license in the practice state
• Active and unrestricted driver’s license with reliable transportation
• Ability to navigate varied home environments and perform assessments

Responsibilities

• Conduct comprehensive in-home assessments for Medicare Advantage and other plan members
• Document diagnoses and care management plans
• Communicate findings to PCPs to address gaps in care
• Identify urgent or emergent situations and intervene appropriately
• Educate members on disease processes, medications, and adherence
• Address social determinants of health and provide referrals as needed
