UnitedHealth Group's Optum HouseCalls team is seeking an Advanced Practice Clinician (NP/PA) to conduct annual in-home health assessments for Medicare Advantage and other plan members. This non-prescribing, field-based role focuses on education, gap closure, and collaboration with primary care providers.

We offer a generous sign-on bonus, flexible scheduling between 8am-8pm across a 7-day week, no on-call, weekends or holidays, and a comprehensive benefits package with 401k.

Qualifications

• Active clinical assessment skills for home health visits.
• Ability to educate patients and families during in-home visits.
• Experience with geriatric or Medicare populations preferred.

Responsibilities

• Conduct comprehensive in-home assessments for patients.
• Identify and document diagnoses for care management and treatment planning.
• Communicate findings to primary care providers to close care gaps.
• Recognize urgent or emergent situations and intervene as needed.
• Educate patients on disease processes, medications, and adherence.
• Address social determinants of health and provide referrals.
